About Your Visit
New PATIENT forms
Please fill out your Health History Questionnaire and sign the consent forms prior to your visit. You will receive these via e-mail when you book your appointment. You may also arrive 20 minutes early to fill out the forms before your appointment time.
Your appointment
Your initial visit will be focused on your health history, your health concerns and goals, and determining your needs. This appointment will be 75-90 minutes. Your initial visit includes acupuncture, and any necessary extras such as cupping, tui na (Chinese medical massage), moxibustion (heat therapy), essential oils, etc. We may prescribe an herbal formula or discuss lifestyle changes and dietary modifications to further support your health.
Try not to come overly full or hungry. Wear loose, comfy clothing so that the points around your elbows and knees are easily accessible.
Follow-up appointments are recommended to establish a treatment plan, and are generally one hour (dependent on modalities used). We will make referrals to other practitioners for specialized or additional care, as needed.
Ask questions! Chinese Medicine is a completely different way of looking at the body. Please feel welcome to ask questions at any time.
Insurance
We do not bill insurance, but many of our patients are able to be reimbursed for our services by submitting a “superbill”, and invoice with specific insurance coding, to their insurance provider. We will create this superbill with appropriate diagnostic codes for you to submit to your insurance company for reimbursement.
You may use your flexible spending and health savings account debit cards for our services.
forms of payment
We accept all major credit cards (including HSA/Flex-spend), checks, and cash payments. Payments are to be made in full at the time of service.
We require a credit card on file to hold your appointment.
Cancellation Policy
We have a 24 Cancellation Policy as common courtesy to our practice and other patients. No-shows and appointments cancelled within 24 hours will be charged for half the cost of the appointment.
We will do our best to accommodate late arrivals but please note your visit may be shortened.
